Boys and Girls 5-8 Grade

In order for your son or daughter to participate s/he'll need the following on file before s/he can begin practices:

1. Sports Physical
2. Student Code of Conduct
3. Parent Code of Conduct
4. $30.00 Participation Fee
5. Consent Form/Liability Waiver
